Requirement

To support the health and well-being of UC students, faculty and staff and our communities, the University of California (in consultation with UC Health leadership) has issued a system wide executive order requiring all members of the UC community to receive an influenza immunization before November 1, 2020. For more information refer to the UCOP Flu Vaccine Requirement Notice

Certification

All faculty and staff will be expected to confirm receipt of the flu vaccination for 2020-21 using the Employee Flu Vaccination Requirement Form. Each employee will receive an email with a personalized link to complete the form.

Please complete the certification to: 1) Report receiving your flu vaccine, 2) Submit a medical exemption, or 3) Request an accommodation. Upon completion you will receive a confirmation email.

Exemptions

If you require a medical exemption, you must download the medical exemption form below and have it completed by your healthcare provider.

Any request for a medical exemption, or request a medical/disability or religious accommodation will be routed to the appropriate office for review and follow-up.  If you already have a medical accommodation document from your physician, you will be required to upload it as part of this process. Information on reasonable accommodations can be found on the Human Resources Reasonable Accommodation website. For more information, refer to the Flu Vaccine procedures.
